Use this form to request reimbursement for expenses incurred on behalf of the Turtle Lake PTA. Send the request along with images of original receipts to Leslee Maki at lesmaki@msn.com and cc:TurtleLakePTA@gmail.com
Special Funding Requests:
Teachers: Submit this form for any requests after using the classroom enrichment and grade-level funds. Email request to TurtleLakePTA@gmail.com or bring it to our PTA meetings (see calendar.)
